These terms explain how FormWiz may be used, what customers are responsible for, and the limits of our liability.

By creating an account, purchasing a subscription, using FormWiz, or accessing any FormWiz service, you agree to these Terms and Conditions. If you do not agree, you must not use the service.
FormWiz provides software tools for creating, publishing, managing, and analysing forms, submissions, documents, workflows, and related content. We may add, change, suspend, or remove features from time to time.
You must not use FormWiz to break the law, infringe third-party rights, distribute malware, send spam, harvest data unlawfully, attempt unauthorised access, disrupt the service, or collect information you are not legally permitted to collect.
You remain responsible for the forms, files, prompts, submissions, documents, messages, and other content you create, upload, collect, or process through FormWiz. You confirm that you have all rights, notices, consents, and lawful bases needed to use that content with the service.
You are responsible for deciding what personal data you collect through your forms and for complying with applicable privacy, data protection, cookie, marketing, recordkeeping, and industry-specific rules. You should not collect highly sensitive data unless you have confirmed that your use case, configuration, notices, retention process, and legal basis are appropriate.
FormWiz may provide AI-assisted form generation, text, calculations, scripts, or suggestions. AI output may be incomplete, inaccurate, unsuitable, or require review. You are responsible for checking, testing, and approving all AI-generated content before publishing or relying on it.
Paid plans, renewals, credit packages, and other purchases are charged according to the prices and billing terms shown at checkout. Unless stated otherwise, fees are non-refundable except where required by law. You are responsible for any taxes, duties, bank charges, currency conversion costs, or payment provider fees that may apply.
Refund requests must be submitted to FormWiz support. Approved refunds are processed by FormWiz through the original payment method, usually via Stripe, and may be reduced by reasonable non-recoverable costs in providing the service, including AI usage, cloud infrastructure, storage, processing, or third-party service fees, to the extent permitted by law. If you request a refund within 28 days of the relevant charge, we may deduct those reasonable costs from the refund where permitted by law.
FormWiz is a software platform. It does not provide legal, financial, tax, accounting, medical, clinical, insurance, compliance, or other professional advice. Any templates, examples, automations, or AI-generated outputs are for general software use only and must be reviewed by appropriately qualified professionals where needed.
To the fullest extent permitted by law, FormWiz is not liable for financial losses arising from your use of, or inability to use, the service. This includes loss of revenue, profit, business, savings, opportunity, goodwill, contract value, investment value, expected benefit, or financial outcome, whether direct, indirect, consequential, incidental, special, punitive, or otherwise.
To the fullest extent permitted by law, FormWiz is provided without warranties of uninterrupted availability, error-free operation, fitness for a particular purpose, accuracy, completeness, or suitability for your business needs. FormWiz will not be liable for losses caused by service interruptions, data loss, corrupted submissions, user error, third-party services, integrations, security incidents outside our reasonable control, or decisions made using information collected or generated through the service.
Where liability cannot legally be excluded, our total aggregate liability for all claims relating to the service is limited to the amount you paid to FormWiz for the service in the three months before the event giving rise to the claim, or GBP 10 if you have not paid for the service.
You agree to indemnify and hold FormWiz harmless from claims, losses, costs, damages, liabilities, and expenses arising from your content, your forms, your submissions, your breach of these terms, your unlawful use of the service, or your failure to obtain required notices, permissions, or consents.
FormWiz may integrate with payment processors, email providers, hosting platforms, analytics tools, AI providers, and other third-party services. We are not responsible for third-party platforms, outages, terms, prices, security practices, data handling, or changes.
We aim to operate FormWiz with reasonable technical and organisational safeguards. No system is completely secure or immune from failure. You are responsible for exporting, backing up, and retaining copies of business-critical submissions, files, and documents where appropriate.
We may suspend or terminate access if we reasonably believe you have breached these terms, failed to pay fees, created risk for the service or other users, or used the service unlawfully. You may stop using the service at any time, subject to any active billing commitments shown at checkout.
We may update these terms from time to time. The updated version will be posted on this page. Continued use of FormWiz after changes are posted means you accept the updated terms.
